This document is required to process a change to an order more than ten (10) business days following sale. Please note that this includes:

  • Change Of Shipping Address
  • Cancellation in Part or in Full
  • Change of Hardware Specification
  • Change of Software Specification
  • Change of One Line Diagram
  • Any other material modifications to the original sale

Please complete this form, print a copy, and sign the document (an electronic signature is not acceptable). Scan your signed document and email to support@deckmonitoring.com, or fax the signed document to DECK Monitoring at 541-636-0275.
